Patients who need regular repeat medication and have had
this approved by a Doctor will have details of their medication
entered onto our computer. When a prescription is issued, the
patient should retain the right hand side of the prescription
form to use as their next request.
When requesting items, please tick the relevant box even if you
only have one item on the slip. This confirms you are requesting
the medication and avoids confusion.
A single request for all the items over several days diverts
staff from other important duties unnecessarily.
If you wish your prescription to be sent directly to the
pharmacy, please tick the relevant box.
The following pharmacies will collect the prescriptions from the
Health Centre:

- Alliance Pharmacy (formerly Moss) Goldsworth Park
- Boots The Chemists, Woking
- Bridge Pharmacy, Woking
- Sainsburys Pharmacy, Brookwood

A stamped addressed envelope must be enclosed
if you wish the prescription to be posted to you.

HAVE PREVIOUSLY OBTAINED THE NECESSARY ACCESS CODES. PLEASE
ASK AT RECEPTION FOR DETAILS.

accept prescription requests over the telephone to avoid the
possibility of misunderstandings.
Please make your request in good time to allow at least two
working days and make allowances for weekends and Bank Holidays.
If you need to request your medication earlier than usual,
please give the reason e.g. if you are going on holiday.
