SICKNESS CERTIFICATES
Under current legislation, a patient ‘self-certificate’ is used
for the first six consecutive working days of any illness. The
self-certificate (Form SC1/2) is available from the Health
Centre, your employer or the Post Office. We do NOT normally
issue a Doctor’s Certificate for this period. By all means show
this statement to your employer if necessary. If an employer
insists on receiving a Doctor’s Certificate to cover the first
seven days of any illness, a charge will be made.
After the first week certificates are obtained as part of a
consultation from your Doctor. If you are under the care of a
hospital Doctor either as an in-patient, you should ask the
Doctor at the hospital for your certificate.
REGISTRATION
In order to qualify for registration you need to be a resident
in the practice area for at least three months. If you have been
here for less than three months you may be seen as a temporary
resident.
To register at the practice, you will need to complete the
registration forms available at the registration desk and all
new patients aged five years and over will be offered a new
patient health check with a clinical assistant.
ACCESS FOR THE DISABLED
The Building is purpose built and all patient services are on
the ground floor. There are access and toilet facilities for
wheelchair patients.

Persons using the footpaths and car park do so at their own
risk. We cannot accept responsibility for injury to pedestrians
or for any loss or damage to vehicles, their contents or
accessories, or any injury caused by or to owners or occupants
of such vehicles whilst entering or leaving the premises.
